Employer Required Notices
These notices listed are required to be provided to employees. Contact our compliance department if you are in need of one of these notices at compliance@davevic.com.

CHIPRA Notice
Employers that maintain group health plans in states that provide Medicaid or SCHIP assistance in the form of premium assistance subsidies are required to give written notice to their employees, informing them of the potential opportunities for premium assistance currently available in the state in which they reside to help them pay for group health coverage. The Employer CHIP Notice is required to be provided automatically on an annual basis.

Summary of Benefits and Coverage (SBC)
The Affordable Care Act (ACA) added a new disclosure requirement for group health plans and health insurance issuers—the summary of benefits and coverage (SBC). The SBC is a short document providing simple and consistent information about a health plan’s benefits and coverage. It must be provided free of charge at certain times, such as at open enrollment.

COBRA - Initial General Notice
One of the basic requirements of Federal COBRA is the requirement to provide an initial notice to covered employees and covered spouses. The initial notice must be furnished by the Employer to the covered employee and spouse when plan coverage first begins. The initial notice only needs to be provided one time when the employee initially receives coverage. Please contact our office for the most current version of the Notice.

Creditable Coverage Notice
Creditable Coverage is prescription drug coverage be at least as good as or better than the standard plan design offered by Medicare Part D. CMS recommends notice be given to all employees regardless of status. This will notify those Medicare eligible spouses and dependents that the employer may not be aware of. Annual requirement beginning of the insurance benefit plan year (renewal/open enrollment).

Disclosure to CMS
Access to Centers for Medicare & Medicaid Services (CMS) online web site for Entities that are required to provide disclosure pertaining to Medicare Part D and Creditable Coverage. Annual requirement within 60-days after the beginning of the insurance benefit plan year.

HIPAA Special Enrollment Notice
This notice is to be provided to make certain that employees understand their right to apply for group health insurance coverage. They should read this notice even if they plan to waive health insurance coverage.
